Depending on what OS you use. I'd use Super Web Cam. Google Super Web Cam

Next sign up for an account on ustream. go turn on super web cam, and get it running, then get into Ustream and logged in and press broadcast now. But first you need to make a channel. which can be found under My Shows at the top. I could show you how to do it by doing a ustream of it. though the quality is horrible since i still never downloaded the encoder thing yet r.r
