I've only seen a few of them, the other being Ute Lady's Tresses: http://www.furaffinity.net/full/11370996/
I remember that these (Goodyeara) were being sold in little bags at a store once. It may have been Lowe's hardware, sold with ferns in little bags too... I bought several of the orchids and ferns and grew them as houseplants and in my terrarium for a while... They did not do horribly well, I think that fewer than half of them were alive two years later, and none four years latter when I moved...
These guys grow in well-drained, acidic soil underneath Douglas Fir trees, in heavy shade and with tons of leaf and bark mulch. I've found them on dry, south and east facing slopes. They're in areas that get a lot of snow in the winter, and not much moisture further onward.
