This one is based on the sardinian folkloristic figure of "S'Accabadora" (literally "she who terminates")... legend says she'd be called by a terminally ill person (or their relatives) for euthanasia. Basically, a sort of local grim reaper. while there is no historical confirmation about this, S'Accabadora is widely represented as a folkloristic figure in local celebrations.
